I probably wouldn't state a price for each maintenance job but put an hourly rate plus materials and if necessary an emergency call out charge (alot of the jobs will be routine maintenance so won't incurr the call out).
In your tender add
Opening letter
company info - usually about three pages long giving brief detailes about yourselves eg: registared address, solicitors, insurers etc then in brief your rules on training, h&S, coshh, risk assessmemts, security, tupe,timekeeping etc.

Of course other relevant things can be added depending on the clients requirement but this is usually the basics.
