OCS story

Hello I have been googling for the ocs accident from earlier this year and cannot seem to find it.

I think it was something to do with the death of a window cleaner in Hull ,whos picture was published in a window cleaner.

Can anyone point me to a link to the story or the posts which was put on here or another forum earlier this year.

That report is from a recent court case where the accident was a few years ago.

That is the right post , thanks for that, what i would also like is to find the report on the internet somewhere.
I think OCS may have put a media block on the accident until there as been a full investigation.

Doesn't this OCS case just say everything about how seriously H&S is taken when the court costs outweight the actual fine itself!!

Surely it should be a much heavier fine??
