Any chance of the last 3 putting their ones up then as Matt has at least put his up but then straight away as per the norm on here people then pick it apart 
I don't have one otherwise I would. I was just agreeing that I think it shouldn't mention about them wishing to cancel or using the word 'unfortunately' - they're negatives.
I think its a tricky one to get right actually, and perhaps it works fine - after all its only one customer at a time who reads it - whereas we're thinking 'oh no its going to hundreds of customers'.
In the main I think its very well worded and displayed but if I were to copy it I would change it slightly as mentioned above, thats not really picking anyone apart, just a point of view.
