Yes, I've backed up a garden straight into a pond. Couldn't believe the weeds all over me. Was like a swamp thing getting out.

Have had a couple of near misses but its only a matter of time.
Clothes lines are my worst enemy for some reason they are always at throat height and they
get you while the customer is watching.

Almost fell in a paddling pool once, it was quite deep with water and in order to avoid falling into it, I had to put a foot right in! ;D

Less worried about falling in - more worried about getting eaten !
Had to walk along the causeway to reach the skylights...

More than once , my last was last June 2012 where my mind was else where (women) lol and I'm in some back garden with a swimming pool and I stepped back without thinking and in a slit second my foot just stepped back and went down and I fell back into a swimming pool nearly upto my waist and I lept out straight away all without letting go of my slx pole I was not going to let that baby break shame i couldn't save my I PHONE in my trouser pocket i was prob in the water no more than 2 seconds , customer was not in so took my socks and trousers off and rang them out and put them back on as it was raining so everything was wet and i then finished off that job and then a conservatory roof clean before I went home and called it a day THAT'S WHAT I CALL PROFESSIONAL lol

A professional wouldnt've wasted time wringing out his socks.
Just extend the pole further and crack on.
Time is money!
