Company Vehicle - Duty Of Care Policy?

Does anyone have one of these I could see please?

Searched the net, but can't find what I want.

Basically a statement your employees would sign, to say they will drive safely and according to road conditions, liable for any speeding or parking fines while vehicle is in their care, will keep it clean, point out any faults, liable for excess etc etc

Got to admit, I've verbally told the lads that when the vehicles are in their care, they are responsible, but I want something to button it down a little more official, so they can't have any excuses.
