Have u not your own method statement you could incorporate it into.?- you could put falling from the ladder as the risk and then use of harness,eyebolts, and correct lifting technique as the control measure.

Yes i have risk assessments, but it would need to be a big ladder because its a 20ft building.

I think you must mean 20 metres.....

Speak with whoever you trained with. They'll have a template you can work with.

If you already have risk assesments then you have what you need. Your risk assesment will of identified the use of harness and eyebolts as a saftey/precautionary measure. There is no such thing as a specific risk assesment/method statement for harness and eyebolts. Any persons using them should be trained in their correct and safe use.
