Accidents

If health n safety Rules r follwed while working at high buildings  then why still 10 men die every year in uk. What could be reason

1. Accidents, sometimes they just happen. 
2. Health and safety procedures not being carried out correctly.
3. Incorrect or no training

I'm sure there is more but there's a few possible reasons
