dont know how to explain but i will try ,my left hand was to high up the pole and my hand was on topside of pole ,muscles were straining felt the difference as soon as i brought my hand down the pole and used an underhand grip my left shoulder relaxed.also by standing one leg in front and rocking a bit takes the strain off th :oe other shoulder.seems to have worked for me
I am the opposite and have used a cupping grip with my left hand for 12 years, after having RSI in left shoulder I found that I could get some slight relief from changing to an overhand grip resting my arm on the pole and letting it slide through my palm. But maybe its IMPORTANT to vary your grip/stance also shrug/rotate your shoulders to keep the body moving.

richard,
i have been having trouble for 18 months or so as i said.but i am getting better slowly.
massage helps  me.but i also have a bit of help now as well,i know this costs money ,but u probably need rest in order to heal.just lifting the hose reel in and out was very  painfull.standing in one position ,bending everything was hell.
i think i still could probably do with a month off and strengthening of core muscles (as trippy says)but its expensive to have a month off.
at one point the dr gave me diclofenic which help also ,but not ongoing.
good luck m8
email me if u want to discuss further as i kno where u are coming from driscolleddie@yahoo.co.uk
