Hi,  does anyone knows where I can get the Above sheets form ? Perhaps you could send it over to me ?
Any help will be greatly appreciated.

Just make your own ,  put your name at the top list customer details, type of carpet or soft furnishing,  work required, area, type of soiling, type of sub floor, then list your terms of business, include a disclaimer that you will not be liable for damage that MAY occur during the cleaning process or use of cleaning agents, unless caused by lack of care by operative.   I have a template that was given to me by one of the companies that trained me, if you do a training course you could ask them if they have a form they could let you have.

We designed our own in excel a good 12 years ago. We knew someone who taught excel so told him what we wanted and he designed a carpet upholstery leather rug 4 survey forms in total.

Type in the sizes and it worked out the price.

Get a book on excel.

The manual you get from Derek who does the training, contains a template for the survey form. I used that for my first few jobs but just crossed out the name of his company and wrote mine.  You will learn alot from Derek, I did a course with him last year. I had previously done one with John from Restormate, who I consider as my mentor. I'm always asking his advice on stuff and he's always happy to help, without him I would be totally out of my depth. I did the second course with Derek as I wanted to further my knowledge of the job.
