How do you get an old membrane out of the casing, seems bloomin awkward, or when you order a new membrane does it come with the case?

It is 'bloomin awkward'. The best way is to use a pair of snipe-nose pliers and pull the centre plastic piece hard.

The new ones do not come with a new housing generally.

As you will be dumping the old membrane don't worry about knackering it, just don't damage the housing.
Any pair of pliers should do it. Grip it and whilst turning the membrane clockwise and anti clockwise gently pull at the same time.

If you have a hole in the plastic hole in the centre part of the membrane, then a screwdriver in the hole levered against the edge of the membrane housing, makes it easy.
