Title: template checklist

hi guys,will you help out please trying too do a check list and im rubbish @ it.
want a checklist for lads to look @ when finish jobs and simple tick of boxes  to show they have checked their work and further down a page to tick if all equipment ie bucket etc in van.
appreciate it if someone could do up one.

Thanks very much for that template it was brillant just what i was looking for and really professional looking. ;)
