Hi James, i belive the NCCA sell em in pads, i use my own but hear the NCCA ones are very good. I use my own becuase i belive everyone likes to taylor things to thier own preferences. I would suggest you use something like MS Publisher and create one taylored around your own methods, yes it will take you a couple of hours but once its done its done and you can tinker with it to get it perfect. MS Publisher is also create for other stuff such as letterheads, postcards, evnelopes even flyers and promotional documents imho.

Do you just have a survey for the custy or do you keep a copy?
If so how do you do it with your own printed pages. (other than using carbon paper and a pritt stick like wot I'm doing atm ::) )
-

I use carbon paper at the moment, but im fiddling with a PDA i bought of ebay and a portable injet , i have a spreadsheet that i use to calculte cost (on the odd occasion i have took my laptop to a quote i use this) im going to use this on the PDA should look very proffessional and the PDA will be my portable diary too.

You should always keep a copy of the survey form so that you have a written record of what you have seen and discussed (hopefully) with the client.
The pre-clean survey plus the qualification with the client is 'the' most important part of the job

You don't really need to give the customer a copy. As long as you have a copy of any defects and it states that the customer has been made aware of potential problems you have highlighted. They then sign it giving you the go ahead, you just keep it safe if case something rears its ugly head a bit later on.

I think that if you don't give the customer a copy of the survey form in the initial stages it could work against you at a later stage if there is a disagreement
