Who do you use, and how much do you pay for one team member?

I used a company called digital risks.

I paid £17 per month for myself and one employee. In fact it might have been £12 per month. Can't remember something like that.

But I wouldn't recommend it, its cheap and probably doesn't cover anything much.

I just got it to satisfy the minimum legal requirements.
