Title: Risk assessment and method statement?

I have been  asked to send a risk assessment and method statement to a children's home before I can book a date for cleaning carpets. I have never done this before so if anybody could send me a template/copy or give any advice it would be really appreciated. I am a dry fusion operative but anything related to carpet cleaning would be helpful.  tlay@talktalk.net     Paperwork.....don't you just love it?
