Richy I don't have one in an email format but I can explain what the ones I have are like

Really simple a5 card format

One side just the name of the premises address etc...

Mine has the months that they are due so April June aug ... Bi monthly etc..

Then the spec e.g clean all external glazing

If its outs and ins it say clean both sides of external glazing etc..

Then on the reverse its divided In 12 boxes where whoever signs the work off ..
