My previous Axeon HF5 membrane lasted over 6 years.  11 months is a joke.

Ive just installed a new Axeon (bought at the same time as the 11-monther).  TDS is now 7ppm from the membrane, so I now know for sure that there's nothing wrong with my setup, it's the membrane that's the problem.
